Enroll
To enroll in a pooled trust with ICS Trust Services, follow the steps below
Step 1: Complete the Trust Application
- Fill out the ICS Community Trust Joinder Agreement and have it notarized.
- The application for Community Trust I and Community Trust II are identical.
- The application can be signed by the designated beneficiary, Power of Attorney (POA), legal guardian, parent or grandparent.
Step 2: Collect the Necessary Documents
- Proof of Income - For Community Trust II only
- SSA/SSDI/SSI: Social Security Benefit Letter or recent bank statement indicating current monthly benefit amounts received from Social Security Administration.
- Pension/VA Benefits: Statement from Pension Company or VA indicating current payment amounts and how often it is issued.
- IRA Distributions: Letter from the financial institution where the IRA account is maintained stating the RMD amount for the current year.
- Proof of Medical Premiums - For Community Trust II only
- Statement from insurance company stating the premium amount and the time period the premium covers.
- Copy of Medicaid Notice of Decision - if beneficiary is enrolled in the Medicaid program.
- Copy of the POA - if Joinder Agreement is signed by Power of Attorney.
- Copy of Guardianship - if Joinder Agreement is signed by legal guardian.
Step 3: Prepare Your Initial Deposit
- The fee to open an account is $250.00. Your initial deposit shall not be less than the enrollment fee.
- If your initial deposit includes excess income or resources, you must add $250 for the enrollment fee.
- Make your check or money order payable to ICS Community Trust.
Step 4: Mail the original, notarized Joinder Agreement with supporting documentation and initial funding to ICS Trust Services at the following address:
